ConsultISM provides a 21st Century approach to marine consultancy, risk management and solutions to managing safety. When an accident occurs, whether it be a collision, grounding, pollution, personal injury or fatality, for example, and an analysis of that accident is undertaken to look for causal factors, you will almost immediately commence an interrogation of the Safety Management System (SMS) of the ISM Code of the ship operating Company.
In an ideal world that SMS should have created a series of barriers to reduce the risk of human errors occurring - to prevent such accidents. The fact that an accident did occur would indicate that one or more of those barriers failed. There may be understandable reasons why the barriers failed - in spite of the best efforts of the ship operator - To err is human!
The reasons might be because of lack of necessary commitment - If you think safety is expensive try having an accident...! Most accidents are preventable if appropriate pro-active steps are taken - Prevention is better than cure!
