Bridge Team Management Failure causes grounding

The Accident Investigation Board of Norway have uploaded their report on the grounding of the bulk carrier Federal Kivalina in Norwegian waters on October 2008. The vessel was fully loaded with aluminium oxide when she struck a charted island, however no pollution or injury resulted from the grounding. The investigation concludes that there was failure of bridge team management and insufficient voyage preparation amongst other factors. This is a further incident where a pilot was on board but where there appears to have been little in the way of communication or teamwork between the pilot and the rest of the bridge team.
